If ANY of the following is TRUE, do not use ONLINE booking, but call Nova Medical Clinic at 780-448-1166.

  • I have recently tested positive for Covid-19 and I am in quarantine.
  • I am currently waiting for Covid-19 lab test results that were ordered due to contact tracing or because of my symptoms.
  • I have travelled outside of Canada in the past 14 days
  • I have been identified as a contact of someone who tested positive for coronavirus.
  • I have been asked to self-isolate by Alberta Health, the Communicable Disease Control or any other governmental health agency
  • I have one or more flu-like symptoms such as:
    – Fever 38C or higher
    – New cough or worsening chronic cough
    – Sore throat or painful swallowing
    – New or worsening shortness of breath
    – Difficulty breathing
    – Runny nose

I confirm that NONE of the above statements are true and I can proceed to book an ONLINE appointment.
